Managing – whether it’s one’s own or a loved one’s – illness can be time consuming, frustrat- ing and frightening. The shock of a new diagnosis can cause tremendous stress, which in turn can impede a patient’s abil- ity to understand the prognosis and treatment options. Adding to the confu- sion is the fact that many chronically ill and older patients have multiple medical issues, which are treated by different specialists and caregivers, who may not always communicate with each other or with the patient and the family. Aging presents even more challenges, involving many complex questions and difficult decisions. For both long distant and “sandwich generation” caregivers, the stress associated with care-giving can cause strains on personal health and rela- tionships. Geriatric Care Managers pro- vide an elite level of care and work close- ly with the family to evaluate and meet your loved one’s needs. Jacqueline O’Doherty founder of Health Care Connect provides the solu- tions families need. Jackie explains, “Our patient advocates act as a surrogate fam- ily member, monitoring and managing a loved one’s care, throughout every stage of the health care journey. Patient advocates facili- tate communication and understanding among the complex network of physi- cians and caregivers, including hospitals and after-care facilities, helping the patient and family weigh each option and possible treatment outcome. Geriatric care management is a spe- cialty of Health Care Connect. “Because so many of our clients are seniors it has been a natural progression to add geri- atric care management to our services.” said Jackie. “It is vital that all the doctors involved are speaking to each other and on the same page, providing patient cen- tered care. HCC also researches appropriate residential facili- ties for senior living and oversees care either at home or in the senior facility. Jackie also works closely with other senior care providers in the community and has the resources to make trusted referrals to eldercare attorneys, daily mail and money managers, financial advisors, senior realtors, assisted living facilities, skilled nursing homes, and companies who specialize in downsizing and transitioning.
