WHAT AGE SHOULD I BRING
MY CHILD IN FOR THEIR
FIRST ORTHODONTIC CHECK-UP?

We have all heard the expression
“Timing is everything,” and this
saying certainly applies to the field
of orthodontics. I often get asked
the question by moms at the local
supermarket, “why do some kids
seem to get braces so early nowa-
days like 7-9 years old and others
not until around 11 or 12?”
Well the best answer is really the
simplest, every child’s orthodontic
case really is different. The guide-
line set by The American Associa-
tion of Orthodontists(AAO) is that
every child should receive an ortho-
dontic examine by the age of 7.
There are several reasons for choos-
ing this age.
The 6 year molars should have
erupted in the back of the mouth in
all four quadrants setting what is
essentially the foundation for the
bite to be built upon as the rest of
the permanent teeth erupt. By age
7-8 the front of the mouth normal-
ly has the four permanent incisors
erupting into place on the upper
and lower jaw, which sets up the
front bite or what we call the over-
bite relationship. As these 12 (out
of 32 total) permanent teeth erupt,
the orthodontist can evaluate the
developing teeth and jaws for sev-
eral important problems they may
possibly need some type of early or
interceptive treatment to prevent
the development of a more serious
problem.
Some examples of more severe
problems at this age include:
• teeth too large for size of jaws
(orthodontic crowding)
• top molar bites inside bottom
molar (posterior crossbite)
• Severe horizontal overlap of
front incisors (overjet)
• Severe Vertical overlap of front
incisors (deep/traumatic overbite)
• Top jaw too narrow for bottom
jaw (transverse maxillary deficiency)
• Top jaw outgrowing bottom jaw
(class II skeletal pattern)
• Bottom jaw outgrowing top jaw
(class III skeletal pattern)
The majority of the kids coming in
for orthodontic screenings at this
age do not present with problems
requiring immediate treatment.
They are placed on an observation
recall to monitor the future growth
and development of their teeth and
jaws to assess any future orthodon-
tic needs. When early or stage one
treatment is indicated for these
types of problems the type of treat-
ment is individually tailored to the
patient.
Some examples of early or stage I
orthodontic treatment include:
• palate expansion appliance – for
posterior crossbite, narrow upper
jaw, crowded teeth, etc
• functional or orthopedic appli-
ances to correct the skeletal mis-
alignment of the top and bottom
jaw
• partial set of braces - severe over-
bite, crowding, spacing
• habit control appliance - thumb
or finger sucking habits
• space maintenance appliance -
missing baby teeth
